Workshop Consent

By signing this form, I acknowledge that I am voluntarily participating in a spiritual workshop facilitated by the organiser and agree to the following terms and conditions:

1. Photography & Promotional Use

I understand that photographs and/or videos may be taken during the workshop for promotional, marketing, educational, and advertising purposes, including use on websites, social media platforms, printed materials, and other promotional content.

2. Participants Under 18 Years of Age

Participants under the age of 18 must be accompanied and supervised by a parent or legal guardian for the duration of the workshop. The accompanying guardian accepts responsibility for the minor’s participation and wellbeing during the event.

3. Non-Refundable Booking Fee

I understand that a non-refundable booking fee is required to secure my place in the workshop. This booking fee is not refundable should I choose to cancel, fail to attend, or be unable to participate for any reason.

4. Incense, Sage & Herbal Smoke

I acknowledge that the workshop may involve the use of incense, sage, palo santo, resins, essential oils, or other herbs that produce smoke and fragrance as part of spiritual and cultural practices. I understand that exposure to smoke, scents, and airborne particles may affect individuals with allergies, asthma, respiratory sensitivities, or other medical conditions. I accept responsibility for assessing my own suitability to participate and will notify the facilitator of any relevant concerns prior to the workshop.

5. Personal Protective Equipment (PPE)

Where required, Personal Protective Equipment (PPE) will be provided. I understand that the use of PPE is mandatory during any workshop activities where the facilitator determines it is necessary for participant safety. I agree to follow all safety instructions and wear the provided PPE when directed.

6. Assumption of Risk

I acknowledge that spiritual workshops may involve meditation, energy work, ritual practices, movement activities, craft activities, handling workshop materials, and exposure to incense, herbs, and other sensory experiences. I participate voluntarily and accept responsibility for my own wellbeing during the workshop. I understand that the workshop is intended for personal growth, education, and spiritual exploration and is not a substitute for professional medical, psychological, legal, or financial advice.

7. Disclaimer – Entertainment, Educational & Personal Development Purposes

I understand that all information, activities, discussions, demonstrations, readings, spiritual practices, intuitive guidance, tea leaf readings, divination, energy work, and other content provided during this workshop are intended solely for entertainment, educational, personal development, and spiritual exploration purposes. I acknowledge that the facilitator does not provide medical, psychological, psychiatric, legal, financial, or other professional advice, diagnosis, treatment, or services. Any insights, interpretations, messages, or guidance shared during the workshop should not be relied upon as a substitute for professional advice. Participants are responsible for making their own decisions and seeking appropriate professional assistance where required. By participating in this workshop, I understand that any information received is subject to personal interpretation and is not guaranteed to be accurate, predictive, or outcome-based. I accept full responsibility for any actions, decisions, or choices I make based on my participation in the workshop

Participant Declaration

I confirm that I have read, understood, and agree to the terms outlined in this consent form. I acknowledge that I am participating voluntarily and accept responsibility for my own actions and wellbeing during the workshop.

Parent/Guardian Consent (For Participants Under 18)

I am the parent/legal guardian of the participant named above and consent to their participation in this workshop. I have read and understood the terms outlined in this consent form and agree to supervise and take responsibility for the minor during the workshop.
